Uncommon Penis Problems: Spontaneous Ejaculation

The problem

A spontaneous ejaculation is exactly what it sounds like - a release of semen from the penis without any "warning" and often without any corresponding orgasm (although this last may vary from man to man or from one spontaneous ejaculation to another). In many instances, the ejaculation occurs in the absence of an erection or any outward sign of arousal in the penis at all.

Some men report that their very first ejaculation may be of the spontaneous variety, although there is some controversy about this. On a purely technical basis, sometimes nocturnal emissions - ejaculating in one’s sleep - may be considered forms of spontaneous ejaculation; however, doctors tend to treat them as distinct from spontaneous ejaculation.

Embarrassing

As one can well imagine, a man who suffers from frequent spontaneous ejaculation (or even infrequent instances) might find it extremely embarrassing. This is the case whether a man experiences the problem in the presence of definite erections or arousal or in their absence.

One of the things that is frustrating about chronic spontaneous ejaculation is that it is usually difficult to predict. However, studies have not been conducted to test this hypothesis.

In some cases, there may be a medical connection to spontaneous ejaculation. Some medications used to treat attention deficit hyperactivity disorder (ADHD), depression, or psychosis may potentially be associated with this form of penis problem.

When medication seems to be a trigger, trying another drug might be an option. If stress is the likely cause, lifestyle changes to minimize stress or to learn to deal with it better (such as meditation and yoga) might be tried.

Penis Problems: ADHD Medications and Spontaneous Ejaculation

There are a wide range of issues which could legitimately be termed penis problems, from erectile dysfunction to dry penis skin to a bent penis to premature ejaculation. One of the more unusual (and therefore rare) penis problems is spontaneous ejaculation. This condition can occur in any man, but some men with ADHD (attention deficit hyperactivity disorder) may be more likely to experience it, as a side effect of medication used to treat ADHD.

About spontaneous ejaculation

Some people may assume that spontaneous ejaculation is the same as premature ejaculation, but they are considered two different conditions.

When a man suffers from premature ejaculation, it generally means that he ejaculates earlier in a sexual encounter than he intends. Typically, this is in reference to penetrative intercourse, with the semen erupting from the penis either before it enters the vagina or within a very short time after successfully penetrating. (The International Society for Sexual Medicine defines that very short time as within one minute of penetration.) Premature ejaculation always therefore occurs during a sexual situation.

Spontaneous ejaculation, on the other hand, refers to semen leaving the penis outside the context of a sexual situation and one in which there has been no physical or mental sexual stimulation beforehand.

Nocturnal emission?

Some men might assume therefore that nocturnal emissions – or wet dreams – would qualify as spontaneous ejaculation. However, nocturnal emission may often include physical or mental stimulation, even if the mental stimulation may be encoded in indirect dream imagery.

When a man has true spontaneous ejaculation, it occurs devoid of sexual context (other than perhaps a non-sexually-induced erection). For instance, one case history reports a teen-aged boy with ADHD who experienced multiple spontaneous ejaculations while taking tests in school.

ADHD

In the case of this boy, and of some other reports in the medical literature, the reason for this spontaneous ejaculation appears to be a combination of medication used to treat his ADHD and a feeling of intense anxiety. The boy had been prescribed methylphenidate, a common medication for ADHD. The medication seemed to be effectively treating his ADHD, but with this embarrassing and inconvenient side effect. When drug use was discontinued, the spontaneous ejaculations did as well.

Another drug associated with ADHD, atomoxetine, also has been reported to cause spontaneous ejaculation. One adult male on this medication reported experiencing spontaneous ejaculation frequently, especially following urination – as often as eight times in one day.

Treating ADHD can be challenging, but there are a number of drugs which are approved for use. Such being the case, if a man with ADHD is prescribed a drug which seems to be a likely cause of spontaneous ejaculation, he should bring this to the attention of his doctor and investigate the possibility of utilizing an alternative medication to treat his ADHD. It should also be noted that sometimes spontaneous ejaculation may be a one-time only occurrence, in which case it may or may not be related to medication.

Premature ejaculation is a concern for many men, but another penis function issue is considerably rarer: spontaneous ejaculation. As the name implies, spontaneous ejaculation is a release of semen which occurs without any physical - and sometimes without any mental - stimulation. Not nocturnal emission

Technically, many instances of nocturnal emission - ejaculating while sleeping - can be considered a form of spontaneous ejaculation. If that were to be the case, then spontaneous ejaculation would not be rare but instead would be quite common among adolescent males. However, when doctors speak of spontaneous ejaculation, they tend to consider it separate from nocturnal emission.

Statistics on this issue are hard to come by, but one study found that approximately 1.5% of men experience their first ejaculation through spontaneous means. However, there is a dearth of statistics on frequency of spontaneous ejaculations later in life. In general, it is felt that the incidence in adulthood is less than 1%. Causes

For example, methylphenidate, which is often used to treat attention deficit hyperactivity disorder (ADHD), can sometimes cause spontaneous ejaculations. At least one case study recounts that reboxetene, a drug used to treat depression, caused the condition in a man. And several antipsychotic drugs include the issue as a potential side effect.

In addition, often a man who is withdrawing from opioid addiction may experience spontaneous ejaculations.

Often, however, the issue seems to be related more to anxiety and stress. For example, a man feeling extreme stress over a college exam or great anxiety due to an important work presentation may become a victim of this problem. Some people with post-traumatic stress syndrome may also have the condition.

Finally, in older men, an enlarged prostate may sometimes produce this result.

Treatment

In cases where medication may be an issue, switching drugs can alleviate the problem. If the prostate is the root of the problem, seeing a doctor for advice is recommended.

If stress and anxiety are the problem, seeking help from a mental health professional may be helpful. In addition, practicing relaxation and meditation activities, including yoga, might be beneficial.
